About the role

Provide one-on-one skilled nursing care to pediatric and adult patients in their homes.

Responsibilities

• Administer medications and treatments
• Manage trachs, vents, and feeding tubes
• Monitor patient conditions closely
• Collaborate with an interdisciplinary team
• Support and educate families
• Maintain detailed clinical documentation

Qualifications & Skills

• Licensed Vocational Nurse (LVN) license
• Compassionate and patient-focused approach
• Strong clinical skills in patient care and management of medical devices
• Excellent communication and teamwork abilities
• Ability to support families and educate them on care procedures
