About the role
We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in patients' homes, following care plans and physician orders.
Responsibilities
- Assessing patient conditions and documenting activities
- Updating care plans and reporting changes to supervisors or physicians
- Educating patients and families on disease management and self-care
- Ensuring infection control and maintaining compliance with policies and regulations
Qualifications
- Active, unrestricted state license
- Current CPR certification
- TB testing within the past year
- Reliable transportation
- Prior clinical experience, especially in home health, is preferred
