Responsibilities
- Providing compassionate, patient-centered homecare to children with low acuity needs, such as g-tube, Syngap1, and behavioral issues.
- Delivering high-quality clinical care, ensuring positive outcomes, and collaborating with families and healthcare teams.
- Utilizing electronic documentation via tablets in patients' homes.
- Supporting patients with individualized care plans and maintaining compliance with health and safety standards.
Qualifications
- An active, unencumbered LPN/LVN license in the practicing state, with compact license transfer if applicable.
- Current CPR certification with a hands-on component, with assistance available post-hire if needed.
- TB skin test within the past 12 months.
- Reliable transportation and at least six months of hands-on nursing experience preferred but not required.
Skills and Qualities
- Strong clinical skills, empathy, and excellent communication abilities.
- Ability to work independently and as part of a multidisciplinary team.
