Private Duty LPN/LVN - Adult Trach/Vent

About the role

Provide compassionate, high-quality homecare for adult patients with trach and ventilator needs. Ensure clinical outcomes through skilled nursing interventions, documentation via electronic systems, and adherence to safety protocols. Support patient comfort and safety while collaborating with the healthcare team.

Responsibilities

• Deliver in-home nursing care for adult trach/vent patients
• Manage trach and ventilator needs
• Maintain accurate documentation using electronic systems
• Provide patient-centered support
• Utilize technology for documentation
• Communicate effectively with healthcare team members

Requirements

• Active, unencumbered LPN/LVN license in the practicing state
• Current CPR certification with hands-on component
• TB skin test within the last 12 months
• Reliable transportation

Qualifications & Skills

• Preferred: Six months of hands-on nursing experience
