Role Overview

The Home and Community Based Services Licensed Practical Nurse provides direct nursing care to acutely and chronically ill patients in their homes, under supervision.

Key Responsibilities

• Deliver high-quality clinical services within legal scope, following care plans and infection control protocols.
• Document patient visits thoroughly in electronic medical records and visit verification systems.
• Participate in care coordination, report patient needs and changes, and communicate effectively with patients, families, and healthcare team members.
• Adapt to various home environments regardless of patient background.

Qualifications & Skills

• Current, unrestricted LPN licensure in the state of practice.
• Valid driver’s license, vehicle insurance, access to transportation, or public transit.
• Preferred: CPR certification, flexible hours, multitasking ability, independence, and strong communication and organizational skills.

Compensation & Benefits

• Hourly pay ranges from $22.90 to $34.35, with comprehensive benefits, incentives, and development opportunities.
