About the role
Provide direct nursing care to acutely and chronically ill patients in their homes, following physician prescriptions and documenting clinical progress.
Responsibilities
- Deliver high-quality clinical services within the scope of practice
- Coordinate care with the healthcare team
- Ensure proper documentation in electronic medical records
- Actively participate in care coordination
- Communicate patient needs and changes effectively
Requirements
- Current unrestricted LPN licensure
- CPR certification or ability to obtain within 90 days
- Valid driver’s license with vehicle insurance or access to transportation
- Ability to work in diverse home environments
