About the role

The Home and Community Based Services Licensed Practical Nurse provides direct nursing care to acutely and chronically ill patients in their homes, following physician prescriptions.

Responsibilities

• Documenting patient visits
• Coordinating care activities
• Ensuring high-quality clinical services within the scope of practice and state laws
• Communicating effectively with patients, families, and healthcare team members
• Participating in care coordination
• Maintaining compliance with infection control procedures

Qualifications

• Current, unrestricted LPN license
• Valid driver's license
• Reliable transportation

Preferred skills

• CPR certification
• Ability to work flexibly
• Ability to manage multiple tasks independently
• Strong communication and organizational abilities
