Private Duty LPN

Role Overview

The Home and Community Based Services Licensed Practical Nurse provides direct nursing care to acutely or chronically ill patients in their homes, under supervision.

Responsibilities

• Deliver high-quality clinical services within the scope of practice, following the plan of care and infection control standards.
• Accurately document patient visits and clinical notes in electronic records and visit verification systems.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, and healthcare team members.
• Provide care in diverse home environments, respecting patient diversity and needs.

Qualifications & Skills

• Current, unrestricted LPN licensure in the practice state.
• Valid driver's license, vehicle insurance, or access to transportation.
• Preferred: CPR certification, ability to work flexible hours, manage multiple tasks, work independently, and possess strong communication and organizational skills.
