About the Role

The Home and Community Based Services Licensed Practical Nurse provides direct nursing care in patients' homes, following physicians' prescriptions and within legal scope. The position emphasizes delivering equitable, quality care regardless of client background and promotes a culture of caring, connecting, and growth.

Responsibilities

• Deliver high-quality clinical services aligned with the care plan and coordinate with the healthcare team.
• Document patient visits comprehensively in electronic records, including visit verification.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, clinicians, and management.

Requirements

• Unrestricted LPN licensure in the practice state.
• CPR certification or ability to obtain within 90 days.
• Ability to work independently in diverse home environments.
• Valid driver’s license, vehicle insurance, or access to reliable transportation.
