About the Role

The Home and Community Based Services Licensed Practical Nurse provides direct nursing care to acutely and chronically ill patients in their homes. They work under supervision to deliver high-quality clinical services, document patient visits thoroughly, and participate in care coordination activities. The role emphasizes effective communication with patients, families, and healthcare teams to ensure optimal patient outcomes.

Responsibilities

• Deliver nursing care within legal and clinical scope, following the care plan and infection control protocols.
• Accurately document patient visits and clinical notes in electronic medical records.
• Participate in care coordination by reporting patient needs and changes, attending case conferences, and communicating effectively with all stakeholders.

Requirements

• Current LPN licensure in the practice state.
• CPR certification or ability to obtain within 90 days.
• Ability to work in diverse home environments and provide compassionate care.
• Valid driver’s license, reliable transportation, or access to public transit.

Skills

• Strong communication, organizational, and multitasking skills.
• Ability to work independently.

Pay

Pay ranges from $20.38 to $36.44 per hour based on experience and location.
