Role Overview

The Home and Community Based Services Licensed Practical Nurse provides direct nursing care to acutely and chronically ill patients in their homes, under supervision.

Key Responsibilities

• Deliver high-quality clinical services within legal and professional scope, following care plans and infection control protocols.
• Thoroughly document patient visits in electronic medical records and visit verification systems.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, and healthcare team members.

Qualifications & Skills

• Current, unrestricted LPN license in the practice state.
• CPR certification or ability to obtain within 90 days.
• Ability to work in diverse home environments and operate transportation reliably.
• Strong communication, organizational, and multitasking skills; ability to work independently.

Pay

Pay ranges from $20.38 to $36.44 per hour, with comprehensive benefits and incentives.
