Private Duty LPN

Responsibilities

• Provide direct nursing care per physician's orders to acutely or chronically ill patients in their homes.
• Document patient visits thoroughly in electronic medical records.
• Participate in care coordination.
• Communicate effectively with patients, families, and healthcare team members.
• Ensure all care adheres to state laws, infection control procedures, and regulatory requirements.
• Adapt to various home situations, regardless of patient background.

Qualifications

• Current, unrestricted LPN licensure.
• Valid driver's license and reliable transportation or access to public transit.

• CPR certification.
