Role Overview

The Home and Community Based Services Licensed Practical Nurse provides direct nursing care to acutely and chronically ill patients in their homes, under supervision of a Registered Nurse or supervisor.

Key Responsibilities

• Deliver high-quality clinical services within legal scope, following the plan of care and infection control procedures.
• Document patient visits comprehensively in electronic medical records and visit verification systems.
• Participate in care coordination, reporting patient needs and changes, and communicating effectively with patients, families, physicians, and team members.
• Adapt to various home environments, ensuring equitable care regardless of client background.

Qualifications & Skills

• Current, unrestricted LPN licensure in the practice state.
• Valid driver's license, vehicle insurance, or access to reliable transportation.
• Preferred: CPR certification, flexibility, independence, strong communication, organization, and multitasking skills.
