Private Duty LPN

About the role

The role involves providing direct, high-quality nursing care to acutely and chronically ill patients in their homes, under the supervision of a Registered Nurse or supervisor.

Responsibilities

• Delivering clinical services within legal and organizational guidelines, following the care plan and infection control protocols.
• Documenting patient visits thoroughly in electronic medical records and visit verification systems.
• Participating in care coordination by reporting patient needs and changes, attending case conferences, and communicating effectively with patients, families, and healthcare team members.
• Providing care in diverse home environments regardless of patient background.

Qualifications

• Current, unrestricted LPN license in the practice state.
• Valid driver's license, vehicle insurance, and access to reliable transportation.

Skills

• CPR certification (preferred).
• Flexibility, multitasking, independence, and strong communication and organizational skills (preferred).

Pay

$22.90 to $34.35 per hour.
