Private Duty LPN

Responsibilities

• Provide direct nursing care to acutely and chronically ill patients in their homes.
• Document clinical notes and coordinate patient care activities.
• Ensure high-quality services within the scope of LPN practice.
• Participate in care coordination by reporting patient needs and changes.
• Attend case conferences and communicate effectively with patients, families, and healthcare team members.
• Ensure compliance with infection control procedures and regulatory documentation standards.
• Deliver clinical services per plan of care and document visits thoroughly.
• Function effectively in diverse home settings.

Requirements

• Current and unrestricted LPN licensure in the practice state.
• Valid driver’s license, vehicle insurance, or access to reliable transportation.

Preferred Skills

• CPR certification.
• Ability to work flexible hours.
• Strong communication, organizational, and independent working skills.
