About the role

Provide direct nursing care to patients in their homes, delivering high-quality clinical services within your scope of practice while following the plan of care and adhering to infection control procedures.

Responsibilities

• Deliver high-quality clinical services within scope of practice, following the plan of care and adhering to infection control procedures.
• Document patient visits accurately in electronic medical records and visit verification systems.
• Participate in care coordination, report patient needs and condition changes, and communicate effectively with patients, families, physicians, and team members.
• Provide care in diverse home environments, respecting patient diversity regardless of age, race, creed, or socioeconomic status.

Requirements

• Current, unrestricted LPN license in the practice state.
• Valid driver’s license, vehicle insurance, or access to reliable transportation.

Preferred Skills

• CPR certification or willingness to obtain within 90 days.
• Flexibility, multitasking ability, independence, and strong communication, organizational, and writing skills.

$22.90 – $34.35 per hour
