About the role

Licensed Practical Nurse providing direct patient care in home settings, under supervision of a Registered Nurse or supervisor.

Responsibilities

• Delivering high-quality clinical services within legal scope, following care plans and infection control protocols.
• Documenting patient visits thoroughly in electronic medical records and visit verification systems.
• Participating in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, and healthcare team.
• Providing care in diverse home environments, respecting patient diversity and needs.

Qualifications & Skills

• Valid LPN licensure in the practice state.
• Reliable transportation and current driver's license.
• Preferred: CPR certification, ability to work flexible hours, multitasking, independent working skills, and strong communication and organizational skills.
