Private Duty LPN

About the role

Provide direct nursing care to acutely and chronically ill patients in their homes, following physician's orders and documenting clinical notes accurately. Coordinate patient care activities, report changes in patient conditions, and communicate effectively with patients, families, and healthcare team members.

Responsibilities

• Deliver high-quality clinical services within legal scope and infection control guidelines
• Thorough documentation of patient visits in electronic medical records
• Participate in care coordination, case conferences, and effective communication
• Provide care regardless of patient background or circumstances

Requirements

• Current LPN licensure in the state of practice
• Valid driver's license, vehicle insurance, or access to transportation
