About the Role

Provide direct patient care in home settings.

Responsibilities

• Deliver high-quality clinical services within legal scope, following the care plan and infection control protocols.
• Thoroughly document patient visits in electronic medical records and electronic visit verification systems.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, and healthcare team members.
• Adapt to diverse home environments, regardless of patient demographics.

Requirements

• Current, unrestricted LPN licensure in the practice state.
• Valid driver’s license, vehicle insurance, or access to public transportation.

Preferred Skills

• CPR certification.
• Ability to work flexible hours.
• Multitasking and independent work capabilities.
• Strong communication and organizational skills.
