Private Duty Licensed Practical Nurse (LPN/LVN)

Responsibilities

• Providing high-quality nursing care to patients in their homes
• Collaborating with a multidisciplinary team to achieve optimal clinical outcomes
• Utilizing electronic documentation tools in patient homes
• Deliver personalized home healthcare
• Ensure patient safety
• Maintain documentation accuracy

Qualifications

• Active, unencumbered LPN/LVN license in the practice state (compact license transfer within 90 days)
• Current CPR certification with hands-on component
• TB skin test within the last 12 months
• Reliable transportation

Hands-on nursing experience preferred but not required.
