Private Duty Home Health LPN Nurse

Role: Private Duty Nurse LPN providing one-on-one skilled nursing care in patients' homes. Responsibilities: Deliver pediatric and adult care including assessments, medication administration, trach/vent and G-tube management, and wound care. Follow individualized care plans, document accurately, and collaborate with families and the clinical team. Qualifications & Skills: Compassionate, skilled in pediatric and adult nursing with experience in trach/vent and G-tube management. Strong documentation, assessment, and communication skills. Ability to work independently and as part of a team. Key Duties: Provide personalized nursing care, ensure patient safety, adhere to care plans, and maintain accurate documentation.
