Responsibilities
- Provide one-on-one skilled nursing care in patients' homes.
- Assess, plan, implement, and evaluate care based on prescribed orders.
- Monitor patient signs and symptoms, identifying physiologic and psychosocial changes.
- Collect, analyze, and document data to determine nursing diagnoses and care needs.
Qualifications
- Licensed as an LPN/LVN in the practicing state.
- Current TB or Chest X-Ray and BLS certification.
- Preferred: at least one year of professional nursing experience.
