Primary Care RN or LPN
About the role
The Primary Care Nurse works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. This role partners with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel. Duties and responsibilities will be performed in a manner consistent with Mercy’s mission, values, and Service Standards.
This is a newly created role for the clinics. The nurse will work closely with Providers and manage triage calls/messages for the clinic. After completing a minimum of three months of on-site training, the role transitions to a 50/50 hybrid model, with full-time (1.0 FTE) co-workers expected to work on-site five days per pay period. Monthly on-site clinic meetings are required.

Qualifications
LPN Qualifications
- Education: Diploma in Practical Nursing
- Licensure: Active LPN license
- Experience: 2-3 years of experience preferred
RN Qualifications
- Education: Associate of Applied Science in Registered Nursing, graduation from a registered nursing program, bachelor’s degree in nursing (BSN), or 4 years of commensurate experience in lieu of the bachelor’s degree
- Licensure: Active RN license
- Experience: 2-3 years of experience preferred
Skills
- Proficiency with Epic EMR system
- Timely and thorough patient triage skills
- Computer competency (most of this position will be computer work)
- Excellent time management and prioritization
- Adaptability in a new and changing environment
Schedule
- Monday – Friday, 0800-1630
- No nights, weekends, or holidays
- Hybrid schedule after three months of on-site training: 50/50 model, with five on-site days per pay period
