About the role
The Primary Care Nurse works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Nurse will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel. Duties and responsibilities will be performed in a manner consistent with Mercy’s mission, values, and Service Standards.
Responsibilities
- Provide patient-centered care in collaboration with primary care providers.
- Perform timely and thorough patient triage.
- Partner with patients, providers, and the clinical care team to drive quality outcomes.
Schedule
- Monday – Friday, 0800–1630 (no nights, weekends, or holidays).
- Three months of on-site training required before transitioning to a hybrid schedule.
- After training, follows a 50/50 hybrid model: full-time (1.0 FTE) employees work on-site five days per pay period.
- Must attend monthly on-site clinic meetings.
Qualifications
LPN Qualifications
- Education: Diploma in Practical Nursing.
- Licensure: Active LPN license.
- Experience: 2–3 years preferred.
RN Qualifications
- Education: Associate of Applied Science in Registered Nursing, graduation from a registered nursing program, bachelor’s degree in nursing (BSN), or 4 years of commensurate experience in lieu of the bachelor’s degree.
- Licensure: Active RN license.
- Experience: 2–3 years preferred.
Skills
- Proficiency with Epic EMR system (highly preferred).
- Strong patient triage skills.
- Computer competency (most of the role involves computer work).
- Excellent time management and prioritization.
- Adaptability in a new and changing environment.
