About the Role
The Primary Care Physician (PCP) provides comprehensive care for older adult patients, managing acute and chronic illnesses, and leading care teams to deliver high-quality, outcome-driven, value-based care.
Responsibilities
- Independently care for patients, seeking specialist consultation as needed.
- Manage and coordinate care for patients in various settings, including hospital, SNF, LTC, or homebound.
- Lead care teams and participate in center management, including recruiting and covering for colleagues.
- Actively engage with hospitalists and transitional care teams for patient management.
- Drive quality improvement, meet clinical and operational metrics, and promote a positive center culture.
Requirements
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar; Board eligibility required.
- Active State Medical License; DEA registration; BLS certification required.
- Proficient in evidence-based medicine, leadership, and team collaboration.
- Excellent communication, time management, and cultural competency skills.
- Proficient in Microsoft Office and able to travel locally and regionally.
