About the role

Primary Care Physician providing equitable, value-based healthcare to Medicare patients in a neighborhood primary care setting.

Responsibilities

• Assess, diagnose, and oversee primary care for patients
• Prescribe medications and manage pharmaceutical treatments
• Maintain Electronic Medical Records using Canopy and Greenway
• Collaborate with regional and central leadership to meet health quality goals
• Work with Practice Managers to direct and manage the care team
• Perform other duties as assigned

Qualifications

MD or DO degree, board certification or eligibility in Internal Medicine or Family Medicine, Active, unrestricted State and DEA licenses, and US work authorization.

Skills

Preferred: Geriatrics fellowship, experience with older adult populations, outpatient primary care, value-based care, bilingual in Spanish. New graduates encouraged to apply.
