About the role

Primary Care Physician delivering equitable, value-based healthcare to Medicare patients in neighborhood clinics. Operates within a team-supported medical center, focusing on quality care, preventive medicine, and positive patient outcomes.

Responsibilities

• Assess, diagnose, and treat patients
• Prescribe medications and manage treatments
• Maintain electronic medical records using Canopy and Greenway
• Collaborate with leadership and care teams to meet health quality goals
• Supervise and coordinate with practice staff
• Perform additional duties as needed

Requirements

• Medical degree (MD or DO) with board certification or eligibility in Internal Medicine or Family Medicine
• Active, unrestricted state license and DEA license
• US work authorization
• Motivated to provide excellent care, especially to older adults

Preferred Qualifications

• Geriatrics fellowship
• Experience in geriatrics, outpatient primary care, and value-based models
• Bilingual skills
