Primary Care Provider

About the Role

The Primary Care Physician (PCP) provides comprehensive care for older adult patients, managing acute and chronic conditions, and leading care teams to achieve high-quality outcomes. They act as a health coach, fostering behavioral changes to improve patient health, while building trusting relationships and simplifying care plans for actionable results.

Responsibilities

• Deliver independent, high-quality care for a patient panel of up to 450 individuals.
• Collaborate with care teams, specialists, hospitals, and transitional care providers to coordinate patient care.
• Lead and motivate care teams, contribute to center management, and support patient recruitment and provider onboarding.
• Monitor and work toward performance metrics such as patient outcomes, experience scores, and clinical gaps.
• Participate in quality and risk management activities, clinical meetings, and uphold compliance standards.
• Conduct patient assessment, diagnosis, treatment, health education, and care coordination, ensuring adherence to clinical guidelines and regulatory standards.

Requirements

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or related field.
• Active state medical license; board eligibility or certification.
• Preferred board certification and current DEA license.
• Strong leadership, communication, and interpersonal skills with a focus on evidence-based medicine and lifestyle factors.
• Proficient in Microsoft Office.
• Willing to travel locally and nationally if needed.
