Primary Care Provider APC

About the Role

We are seeking an experienced Primary Care Provider - Advanced Practice Clinician (APC) to independently manage a patient panel in a collaborative, value-based care environment. The role emphasizes clinical autonomy, team collaboration, and opportunities for career development.

Responsibilities

• Provide comprehensive, longitudinal primary care, including assessments, diagnoses, and treatment for acute and chronic conditions.
• Manage patient care plans, referrals, test results, and medication management.
• Coordinate with interdisciplinary team members and support shared decision-making with patients and families.
• Participate in practice efficiency initiatives and clinical workflows.

Requirements

• Master’s degree with national certification (MSN for NPs or NCCPA for PAs).
• Active, unrestricted licensure in Massachusetts.
• Minimum of three years' experience managing a primary care panel, including preventive, longitudinal, and chronic care, especially for adult patients 65+.
• Experience with inbox management, test follow-up, and care coordination.
• Current BLS certification, DEA, and MCSR licenses or ability to obtain prior to employment.

Preferred Qualifications

• Experience in value-based, population health, or team-based care models.
• Managing complex adult patients with multiple chronic conditions.
• Epic EMR experience and familiarity with clinical pathways and quality initiatives.
