Primary Care Provider (APC)

About the role

Join a healthcare organization as a Primary Care Advanced Practice Clinician (NP or PA) in a collaborative, value-based environment.

Responsibilities

• Manage a panel of primary care patients independently, providing longitudinal, relationship-based care
• Perform assessments, diagnoses, and treatment in ambulatory settings
• Manage acute and chronic conditions, developing and supervising care plans
• Make clinical decisions on in-office management versus referrals
• Handle inbox responsibilities such as test results, messaging, medication management, and referrals
• Coordinate care with interdisciplinary team members, supporting shared decision-making with patients and families
• Provide consultation on hospitalized patients within the panel
• Support practice efficiency, quality initiatives, and clinical workflows

Requirements

• Master’s degree (MSN for NP or equivalent for PA) with national certification
• Active, unrestricted Massachusetts license, DEA, and BLS certification
• Minimum three years of experience managing a primary care panel, including preventive, longitudinal, and chronic care for adults aged 65+

Preferred Qualifications

• Experience in value-based, population health, or team-based care models
• Managing medically complex adult patients
• Epic EHR experience and familiarity with clinical pathways and quality initiatives

Pay

Competitive salary range: $109,500–$164,000
