About the role
Join a healthcare organization as a Primary Care Advanced Practice Clinician (NP or PA) in a collaborative, value-based environment.
Responsibilities
- Manage a panel of primary care patients independently, providing longitudinal, relationship-based care
- Perform assessments, diagnoses, and treatment in ambulatory settings
- Manage acute and chronic conditions, developing and supervising care plans
- Make clinical decisions on in-office management versus referrals
- Handle inbox responsibilities such as test results, messaging, medication management, and referrals
- Coordinate care with interdisciplinary team members, supporting shared decision-making with patients and families
- Provide consultation on hospitalized patients within the panel
- Support practice efficiency, quality initiatives, and clinical workflows
Requirements
- Master’s degree (MSN for NP or equivalent for PA) with national certification
- Active, unrestricted Massachusetts license, DEA, and BLS certification
- Minimum three years of experience managing a primary care panel, including preventive, longitudinal, and chronic care for adults aged 65+
Preferred Qualifications
- Experience in value-based, population health, or team-based care models
- Managing medically complex adult patients
- Epic EHR experience and familiarity with clinical pathways and quality initiatives
Pay
Competitive salary range: $109,500–$164,000
