Responsibilities

• Provide coordinated care for medically complex seniors with the support of nurses, care managers, social workers, specialists, facility staff, and pharmacists.
• Focus on proactive, value-based care and outcomes rather than volume.

Requirements

• MD/DO from an accredited medical school.
• Active Arizona medical license and DEA registration.
• Board certified or eligible.
• Geriatrics, post-acute, SNF, or LTC experience preferred but not required.

Pay

• $211,369–$317,053 base salary.
• $30,000 sign-on bonus.
• Quarterly Incentive Plan — based on outcomes, quality metrics, and documentation.
• Long-Term Incentive Eligible — physicians may participate in the Profit Shares program.
