Qualifications
- Graduate of an accredited School of Medicine, College of Osteopathic Medicine, or accredited Nurse Practitioner program
- Current licensure as a Physician (MD/DO) or Advanced Practice Registered Nurse (APRN) in the applicable state
- Board certification in Internal Medicine, Family Medicine, Geriatric Medicine, or applicable Nurse Practitioner specialty certification preferred
- Minimum of one year of experience working with frail elderly, long-term care, hospice, palliative care, or medically complex patient populations
- Demonstrated ability to work collaboratively within an interdisciplinary team to manage complex patient care needs
Supervisory Responsibilities
This position has no direct supervisory responsibilities. The Physician works collaboratively with Nurse Practitioners and other healthcare professionals as part of the interdisciplinary care team to support the delivery of patient-centered care.
Duties And Responsibilities
- Performs an initial comprehensive history and physical assessment on new enrollees, interval assessments on established participants (semi-annually, as condition necessitates or as requested)
- Functions as a member of the Interdisciplinary Team (IDT), communicates participant changes to team members, and participates in IDT meetings
- Assists in the development, implementation, and evaluation of the participant plan of care as described in Senior Care Partners P.A.C.E. policy
- Evaluates and treats participants with episodic illnesses and documents in the medical record as required by policy
- Provides, promotes, and educates participants and families on preventative health maintenance of their chronic medical conditions and actively participates in family conferences
- Regulates and adjusts participant medications as needed and in a cost-effective manner
- Facilitates consultations with medical specialists as indicated and oversees care rendered
- Collaborates effectively with primary (attending) medical management while an inpatient; Provides updates to the IDT on a regular basis; Judiciously utilizes specialty consultants; Coordinates discharge planning with the IDT and hospital discharge planners
- Maintains all of a participants care if admitted to a skilled nursing facility (SNF) to include: Regularly visit the SNF for participant evaluation; Communicates with SNF staff via phone/fax as requested/required
- Participates in committees, work groups as assigned, and Quality Improvement activities as required
- Participates in the coordination of the participant’s 24-hour delivery of care services
- Maintains current accurate documentation of health care services provided, coordinated or contracted
- Must care, support, and collaborate with everyone involved in participant's care including the participant’s family using the patient-family centered care approach
Physical Demands
- Must be able to move intermittently throughout the workday
- Must be able to cope with the mental and emotional stress of the position
- Must possess sight/hearing senses or use prosthetics that will enable these senses to function so that the requirements of this position can be fully met
- Must function independently, have flexibility, personal integrity, and the ability to work effectively with participants, personnel, and support agencies
- Must meet the general health requirements set forth by the policies of Senior Care Partners P.A.C.E., which includes a medical physical examination
- Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times hostile people within the Senior Care Partners P.A.C.E. Program and/or center
- Must be able to lift twenty pounds
- May be necessary to assist in the evacuation of participants during emergency and/or disaster situations
