Primary Care Physician
About the role
Sage Health believes all seniors regardless of means deserve concierge primary care and wellness without concierge fees. The company builds enriching neighborhood health centers that are easy to access, provide or arrange for all patients' healthcare needs, and partner with Medicare Advantage plans that fully cover primary care. Unlike other physician practices, a Sage Health physician has a smaller patient panel size and collaborates with the best outcomes-oriented specialists and hospitals in each market. Operating under a global capitation budget from Medicare Advantage plans, the organization's sole motivation is to keep seniors healthy.
The Primary Care Physician (PCP) works as a lead in a team-based care model focused on quality of care for seniors. The care team includes physicians, advanced practice professionals, pharmacists, care associates, medical assistants, behavioral health support, quality-based coders, referral coordinators, and more. The model provides a concierge experience, multidisciplinary services, coordinated care supported by analytics and tools, and deep community relationships, allowing PCPs to see fewer patients and spend more time caring for them.
