Primary Care Physician

Role: Primary Care Provider – Physician Responsibilities Deliver comprehensive, patient-centered primary care to diverse and underserved populations.Provide acute, chronic, and preventive care, managing chronic diseases effectively.Coordinate care with behavioral health, dental, and specialty service teams.Utilize Electronic Health Records (EHRs) to document and manage patient information.Contribute to quality improvement and health equity initiatives.Collaborate with an inclusive, mission-driven care team focused on accessible, high-quality community health. Qualifications & Skills Strong clinical skills in primary care, including managing acute and chronic conditions.Experience working with diverse, underserved populations preferred.Ability to work collaboratively within a team-based, community-oriented environment.Proficiency with EHR systems and commitment to quality and equity in healthcare.
