Primary Care Physician
About the role
Lykos Medical is seeking a Primary Care Physician (MD/DO) to provide supervisory and clinical oversight for Long-Term Care (LTC) and Skilled Nursing Facilities (SNF) across rural Southeast Colorado. This role is ideal for physicians seeking schedule flexibility, autonomy, and the opportunity to support underserved senior communities.
Responsibilities
- Provide supervisory oversight and monthly in-person rounding
- Manage chronic and complex geriatric patients in LTC/SNF settings
- Collaborate with facility nursing and administrative teams
- Document care in EMR in a timely manner
- Participate in quality improvement and post-acute care initiatives
- Build strong relationships with rural facilities and care teams
Qualifications
- Required: MD or DO, Board Certified/Eligible in Internal Medicine, Family Medicine, or Geriatrics, Active CO medical license (or ability to obtain), DEA license
- PREFERRED: Experience in LTC, SNF, geriatrics, post-acute care, or rural medicine, Familiarity with Medicare and value-based care, Interest in leadership or future program growth

We schedule around your availability and are travel-friendly, offering compensation based on productivity and flat travel fees or stipends due to long driving distances. We also provide opportunities for leadership and expansion into future LTC/SNF markets in Colorado Springs or Pueblo.
