About the role

Serve a diverse, high-need community where your care directly impacts lives in one of the city’s most meaningful settings. We’re a mission-centered outpatient clinic in the NYC metro area built to deliver whole-person primary care for patients who too often fall through the cracks—people facing housing insecurity, complex medical needs, and barriers to consistent access. Our model is intentionally integrated and team-based: primary care is supported by behavioral health collaboration, care management, and strong operational partners, so clinicians can practice thoughtful medicine with real support, not heroics.

Why join us

Because you’re not looking for “just another clinic job.” You want work that matters and a practice environment where you can do it well. In this role, you’ll have the rare chance to:

  • Help shape a growing clinic’s workflows, care pathways, and patient experience
  • Practice in a setting that values relationship-based, outcomes-focused care
  • Partner with an interdisciplinary team that understands the realities of complex care
  • Step into a position with visibility and an upward trajectory as the program expands
  • Join a stable organization with meaningful resources, while still having startup-level influence

If you’ve been craving a role where your clinical judgment and leadership instincts are welcomed (not boxed in), this is it!

Responsibilities

  • Provide comprehensive patient care, including consultations, evaluations, treatments, and preventive health measures within prescribed guidelines and instructions of the supervising physician
  • Develop and implement patient management plans, recording patient history and physical assessment results
  • Treat chronic health conditions and promote disease prevention
  • Implement medication-assisted treatments and monitor patients’ medication therapies and treatment plans
  • Integrate behavioral health into patient care and work closely with mental health professionals to ensure patients receive holistic care
  • Maintain accurate, detailed reports and records using EMR documentation
  • Collaborate with other healthcare professionals to form a healthcare team for each patient
  • Educate patients and their families about their conditions and what they can do to improve their health
  • Stay up-to-date with advancements in healthcare technology and best practices in family medicine

Qualifications

  • Medical Degree (MD) or Doctor of Osteopathy (DO) from an accredited institution
  • Board Certified or Board Eligible in Family Medicine
  • Valid and unrestricted license to practice medicine in the state
  • Minimum of 2 years of experience in a primary care role
  • Experience in chronic disease management and preventive care for adult populations
  • Proficiency in medication-assisted treatment and behavioral health integration
  • Experience working with electronic medical records (EMR) documentation
  • Excellent communication and interpersonal skills, with a focus on patient education
  • Strong commitment to health promotion and disease prevention
  • Ability to work in a team-oriented environment and collaborate with other healthcare professionals
