Role Overview: The Primary Care Physician provides equitable, value-based healthcare to Medicare patients at neighborhood clinics. Operating within a team-supported environment, they focus on delivering high-quality care, preventive medicine, and improving patient outcomes, with performance rewards tied to results. Assess, diagnose, and treat patientsPrescribe medications and oversee treatmentsManage electronic medical recordsCollaborate with leadership to meet quality goalsLead and coordinate care teamsOther relevant duties as assigned Qualifications & Skills: MD or DO, Board-certified or eligible in Family or Internal Medicine, active unrestricted license and DEA, US work authorization. Preferred: Geriatrics fellowship, experience with older adults, outpatient and value-based care experience, bilingual skills.
