About the role
The Primary Care Physician (PCP) provides comprehensive care for older adult patients with acute and chronic conditions, ensuring high-quality outcomes and cost-effective treatment. They are accountable for patient health, building trust, and acting as a health coach to support behavior change and long-term wellness.
Responsibilities
- Independently manage patient care, including assessment, diagnosis, treatment, and care coordination.
- Lead care teams comprising medical assistants, care facilitators, and coordinators.
- Collaborate with hospitalists and other healthcare professionals for patient hospitalizations and transitions.
- Participate in risk and quality management programs, clinical meetings, and adhere to compliance standards.
- Engage in center management, including recruiting patients and providers when needed.
- Supervise and collaborate with Advanced Practice Practitioners, remaining accountable for their actions.
Qualifications
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or related specialty; board eligibility or certification preferred.
- Current medical license and DEA registration.
- Basic Life Support (BLS) certification required.

- Strong leadership, communication, and organizational skills.
- Proficiency in evidence-based medicine, lifestyle and nutritional interventions, and technology tools.
- Availability to travel locally and nationally as needed.
