Primary Care Physician

About the role

The Primary Care Physician (PCP) provides comprehensive care for patients with acute and chronic illnesses, primarily older adults. They are accountable for patient outcomes, ensuring high-quality, cost-effective care, and act as health coaches to motivate behavior changes. The PCP leads a care team, coordinates with specialists and hospitalists, and manages patients across various settings including in-office, hospital, SNF, LTC, or home. They participate in practice management, recruitment, and fostering a positive center culture.

Responsibilities

• Deliver independent medical care, diagnosis, treatment, and health promotion.
• Lead and supervise care teams, collaborating with other healthcare professionals.
• Engage with hospital and transitional care teams for hospitalized or homebound patients.
• Ensure compliance with guidelines, quality standards, and departmental goals.
• Participate in risk management, clinical meetings, and care coordination activities.

Qualifications

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar.
• Board eligibility or certification preferred.
• Active State Medical License and DEA registration.
• BLS certification required within 90 days.
• Proficient in Microsoft Office and capable of independent judgment.
• Excellent communication skills, availability for patient access, and willingness to travel locally and nationally as needed.
• Strong leadership, influence, and commitment to continuous improvement and physician wellness.
