About the role
The Primary Care Physician (PCP) provides comprehensive care for patients with acute and chronic illnesses, primarily older adults. They are accountable for patient outcomes, ensuring high-quality, cost-effective care, and act as health coaches to motivate behavior changes. The PCP leads a care team, coordinates with specialists and hospitalists, and manages patients across various settings including in-office, hospital, SNF, LTC, or home. They participate in practice management, recruitment, and fostering a positive center culture.
Responsibilities
- Deliver independent medical care, diagnosis, treatment, and health promotion.
- Lead and supervise care teams, collaborating with other healthcare professionals.
- Engage with hospital and transitional care teams for hospitalized or homebound patients.
- Ensure compliance with guidelines, quality standards, and departmental goals.
- Participate in risk management, clinical meetings, and care coordination activities.
Qualifications
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar.
- Board eligibility or certification preferred.
- Active State Medical License and DEA registration.
- BLS certification required within 90 days.
- Proficient in Microsoft Office and capable of independent judgment.
- Excellent communication skills, availability for patient access, and willingness to travel locally and nationally as needed.
- Strong leadership, influence, and commitment to continuous improvement and physician wellness.
