About the role
Provide equitable, value-based primary care to Medicare patients in a neighborhood clinic setting. Collaborate with care teams and regional leadership to meet quality goals, assess and diagnose patients, prescribe treatments, and maintain electronic medical records. Lead and manage clinic care teams, ensuring high-quality patient outcomes.
Responsibilities
- Assess, diagnose, and treat patients
- Oversee primary care delivery and medication management
- Maintain Electronic Medical Records
- Collaborate with leadership to achieve health quality metrics
- Manage clinic care teams with Practice Managers
- Perform other duties as assigned
Qualifications & Skills
- Medical degree (MD or DO)
- Board certification or eligibility in Internal Medicine or Family Medicine
- Active, unrestricted medical license and DEA registration
- US work authorization
- Preferred: Geriatrics fellowship, experience with geriatrics, outpatient primary care, and value-based models; bilingual skills are a plus
