You will provide relationship‑based primary care, respond to acute needs, and drive measurable improvements in health outcomes—all with full support from Bloom’s interdisciplinary team and advanced data systems. Responsibilities

• Serve as the primary care provider for a defined patient panel, delivering longitudinal, holistic medical care in patient homes and facilities.
• Diagnose, manage, and treat chronic and acute conditions with support from clinical operations, triage team, and MD leadership.
• Address social determinants of health, including housing stability, nutrition, caregiver support, and access barriers.
• Conduct specialty visits such as wound care, post‑hospital transitional care, and condition‑specific assessments.
• Complete comprehensive evaluations including medication reconciliation, fall‑risk assessments, advance care planning, and health education for patients and caregivers.
• Utilize Bloom’s integrated data ecosystem to track hospitalizations, coordinate timely post‑discharge visits (within 72 hours), and drive superior outcomes.
• Collaborate closely with internal and external care teams to ensure continuity, reduce readmissions, and stabilize high‑risk patients.

Compensation

$225,000 – $260,000 annually, depending on experience, with opportunity for performance‑based bonuses and full benefits.
