About the role
We are seeking an experienced Family Nurse Practitioner (FNP) for an outpatient primary care role in a patient-centered, collaborative setting.
Responsibilities
- Manage chronic diseases such as diabetes, hypertension, and hyperlipidemia
- Provide geriatric and veteran-focused care
- Work independently with your own patient panel
- Participate in a team-based PACT model with RN, LVN/LPN, MA, and support staff
- Utilize CPRS EMR system
Requirements
- Certification as a Family Nurse Practitioner
- Minimum of 2 years outpatient primary care experience
- Strong background in chronic disease management
- Ability to practice independently
- Veteran care experience is a plus
