Role: Float Registered Nurse supporting primary care centers for adults on Medicare. Responsibilities Rotate between clinics to deliver patient-centered, preventative care.Perform assessments, medication administration, and care coordination.Manage chronic diseases and support care transitions.Collaborate with physicians, nurse practitioners, and care teams to develop comprehensive care plans.Provide patient and family education to promote health literacy.
