Primary Care Collaborative Physician - MD
About the role
Under supervision of the CMO, this clinical physician role involves providing high-quality complex and geriatric care to residents and patients across various assisted living and skilled nursing facilities. The position also includes collaborating with non-physician practitioners and other interprofessional team members.
Responsibilities
- Provides clinical care to residents and patients in partnership with non-physician practitioners and other interprofessional team members.
- Keeps high engagement with facility leadership teams, nurse practitioners/physician assistants, patients, and families.
- Collaborates with nurse practitioners/physician assistants to support their clinical roles based on state regulatory parameters.
- Audits up to 10% of practitioner medical orders and/or documentation for each provider monthly.
- If needed, assumes medical directorships for skilled and/or other senior living facilities.
- Identifies new growth opportunities with skilled facilities and/or other senior living facilities in the market/region.
- Participates in company frontline/backup call processes for timely response to acute changes of condition to prevent avoidable hospital transfers or meet other clinical needs.
- Focuses on quality outcomes including care path compliance, productivity, prevention of avoidable transfers, and reduction in polypharmacy and costs of care.
- Attends required trainings and meetings as mandated by the CMO or other leaders to ensure best care delivery by the company.
Qualifications
- Graduate from an accredited school of medicine and be a Doctor of Medicine.
- Two or more years of administrative leadership experience in healthcare.
- Seven years or more of clinical medical experience preferred.
- Board certification in Family Medicine, Internal Medicine, or Geriatrics; qualification in Geriatrics preferred.
- Currently licensed under the provisions of the State Board of Medicine with active unrestricted licensure to practice in the state of employment.
- Current Drug Enforcement Administration registration.
