Primary Care Collaborative Physician - MD

About the Role

In this clinical collaborative physician role, under the supervision of the CMO, you will have several clinical responsibilities along with specified administrative duties. You will provide day-to-day clinical care to residents in facilities and homebound patients while practicing and role-modeling ethical and evidence-based decision-making to promote the company’s vision and mission.

Responsibilities

• Provides high-quality complex and geriatric care to residents and patients across settings in partnership with non-physician practitioners and other interprofessional team members for an identified panel of patients in assisted living and skilled nursing facilities.
• Maintains high engagement with facility leadership teams, nurse practitioners/physician assistants, patients, and families.
• Serves as a collaborating physician for an average of 5 but up to 15 nurse practitioners and physician assistants (based on state regulatory parameters) to support their clinical roles.
• Audits up to 10% of practitioner medical orders and/or documentation for each provider monthly, based on state and regional regulatory frameworks.
• Assumes medical directorships for skilled and/or other senior living facilities, if needed.
• Helps identify new growth opportunities with skilled facilities and/or other senior living facilities in the market/region.
• Participates in company frontline/backup call processes for timely response to acute changes in condition to prevent avoidable hospital transfers or meet other clinical needs.
• Maintains a high focus on quality outcomes, including care path compliance, productivity, prevention of avoidable transfers, and reduction in polypharmacy and costs of care.
• Attends required trainings and meetings as mandated by the CMO or other leaders to ensure best care delivery by the company.

Requirements

• Graduate from an accredited school of medicine and holds a Doctor of Medicine degree.
• Two or more years of administrative leadership experience in healthcare.
• Seven or more years of clinical medical experience preferred.
• Board certification in Family Medicine, Internal Medicine, or Geriatrics; Geriatrics qualification preferred.
• Active unrestricted licensure to practice in the state of employment.
• Current Drug Enforcement Administration registration.

Pay

Salary Range: USD $200,000.00 - $250,000.00 per year.
