Primary Care Assistant - LPN - PRN, Days

Role: Primary Care Patient Care LPN working under the primary care operations team to deliver safe, effective, and patient-centered care in a systematic manner. Responsibilities Collaborate with patients, providers, and care team members to drive quality outcomes.Perform duties consistent with organizational mission, values, and standards.Provide timely and equitable care in a primary care setting. Qualifications Graduate of an accredited practical nursing program.Licensed Practical Nurse (LPN) licensure in the state of practice.Preferred: 6 months clinical healthcare experience, preferably in primary care. Skills & Abilities Strong team collaboration, communication, and customer service skills.Excellent organizational, interpersonal, and attention to detail.Flexible, positive attitude, and self-directed with the ability to work independently. Physical Requirements Ability to lift, push, pull 50 pounds regularly.Prolonged standing, walking, and performing tasks requiring reaching, bending, kneeling, and twisting.
