About the role

Collaborate with primary care providers to deliver safe, effective, patient-centered care, focusing on quality outcomes and supporting the clinical care team.

Responsibilities

• Assist in patient care activities
• Communicate effectively with patients and team members
• Perform tasks within scope of practice independently with minimal oversight

Qualifications

• Graduate of an accredited practical nursing program
• Licensed as an LPN in the practice state
• Preferred: healthcare experience in a primary care setting
