Primary Care Assistant - Full Time

Role: The Primary Care Patient Care LPN provides safe, effective, patient-centered care in collaboration with primary care providers and the clinical team. Key Responsibilities: Partner with patients, providers, and team members to drive quality health outcomes, ensuring care aligns with organizational standards, values, and mission. Perform duties independently within scope of practice with minimal oversight.Communicate effectively with patients and team members.Support a collaborative, patient-focused care environment. Qualifications: Graduate of an accredited practical nursing program and licensed as an LPN in the practice state. Skills & Abilities: Excellent communication, organizational, and interpersonal skills; customer service orientation; team player; flexible, positive attitude; attention to detail; self-directed. Preferred Experience: At least 6 months of clinical healthcare experience, ideally in a primary care setting. Physical Demands: Ability to lift 50 pounds, stand and walk for prolonged periods, and perform physical activities like reaching, bending, kneeling, and twisting.
