Primary Care Assistant - Full Time

Role Summary The Primary Care Patient Care LPN collaborates with providers and the care team to deliver safe, effective, patient-centered care in a primary care setting. Key Responsibilities Partner with patients, providers, and team members to improve quality outcomes.Perform duties aligned with organizational standards, ensuring safety and efficiency. Qualifications & Skills Graduate of an accredited practical nursing program.Licensed Practical Nurse (LPN) licensure in the practice state.Strong team collaboration, communication, organizational, and interpersonal skills.Customer service orientation with attention to detail and accuracy.Self-directed with the ability to perform tasks independently with minimal oversight.Flexible, positive attitude, and willing to collaborate across the care team. Preferred Experience & Physical Requirements At least 6 months of clinical healthcare experience, preferably in primary care.Ability to lift, push, or pull up to 50 pounds, and perform prolonged standing, walking, reaching, bending, kneeling, twisting, and squatting.
