About the role

The Primary Care Patient Care LPN supports primary care providers to deliver safe, effective, and patient-centered care.

Responsibilities

• Collaborate with patients, providers, and clinical team members to improve quality outcomes
• Perform tasks independently within scope of practice
• Adhere to mission, values, and standards

Requirements

• Graduate from an accredited practical nursing program
• Hold an active LPN license

Preferred Experience

• At least 6 months of clinical healthcare experience, preferably in a primary care setting

Physical Requirements

• Ability to lift 50 pounds
• Stand and walk for extended periods
• Perform physical tasks such as reaching, bending, and kneeling
